New Patient Information
What to expect:
An initial Integrative Acupuncture Care visit lasts about 75- 90 minutes, depending on the complexity of your Condition(s). During the initial visit you will have an extended interview with me to discuss and review your health story. An evaluation and exam of your radial pulses, your tongue, and other areas of the body related to your symptoms will also take place. This initial evaluation will help us to reach a preliminary diagnosis and a treatment plan.*

I may ask you for further lab testing or medical information to make a more comprehensive diagnosis and plan. Following the initial visit I will review your health story in detail and prepare a report of findings, which will propose an integrated plan for your health and healing.
Acupuncture of MarinAcupuncture treatments last about 30 minutes. The treatment often results in a state of tranquility and ease that supports self-healing. During the treatment, you will be in a warm, inviting room on a comfortable table. I keep a monitor in my rooms for my patients to call me if they have any needs.

How to prepare:
• Please wear comfortable clothing that can be pulled up to the knees and past the elbows, because regularly used acu-points are located from the elbow and knee down.

• If you want to save time when you first arrive, you can print out and complete the New Patient Intake Form, the Metabolic Assessment Form, & the Informed Consent & Arbitration Form.

• If you feel I may need extra preparation,  please email me a summary of your health story to Jeffrey@AcupunctureofMarin.com.

• If I request it of you, please also fill out the Traditional Oriental Medicine Assessment Form.

• If you find that you have questions, please don’t hesitate to contact the office.
